Mental Wellness Program

Mental health concerns and substance use often develop side by side. At The Ranch Tennessee, the mental wellness program offers clients a grounded, integrated approach to treating both. The pace of The Ranch—quiet mornings, steady routines, time outdoors—provides a setting where clients can settle enough to examine underlying patterns and begin meaningful therapeutic work.

Co-occurring conditions we support

Many clients arrive with more than one mental health challenge. The Ranch’s dual diagnosis approach treats these conditions together rather than separately. We support clients experiencing:

  • Anxiety
  • Depression
  • PTSD
  • Bipolar disorder
  • ADHD

Treating mental health symptoms alongside substance use helps clients build stability that lasts beyond their time in treatment.

Why dual diagnosis care matters

Clients may use substances to manage overwhelming emotions or untreated symptoms. Likewise, substance use can intensify or mask underlying mental health issues. Treating both conditions together creates clearer, more sustainable progress.

Examples of needs explored here include:

  • Emotional shutdown or hyperarousal
  • Difficulty regulating stress
  • Sleep and appetite disruptions
  • Avoidance patterns
  • Cognitive loops that affect mood or behavior

How the mental wellness program works

Clients participate in a combination of clinical and experiential therapies, such as:

  • Individual counseling
  • CBT, DBT, EMDR, and trauma-informed approaches
  • Medication management when appropriate
  • Equine therapy for grounding and relational awareness
  • Mindfulness, creative therapies, and group work
  • Outdoor activities that encourage reflection and movement

Continued support

After completing programming, clients receive aftercare plans and access to alumni support to help maintain their mental wellness.

Brian Fuller, MD

Medical Director

Dr. Fuller is board certified by the American Board of Psychiatry and Neurology. He completed medical school at the University of Tennessee Health Science Center and residency at Vanderbilt University. Additionally, he has completed the Nashville Advance Psychodynamic Psychotherapy Program, a two-year intensive program to better integrate traditional psychotherapy into his psychiatric practice. Dr. Fuller began his post-residency career in psychiatry running a psychotherapy-oriented private practice. Looking to help more people than he could working alone yet still provide treatment that emphasizes the importance and value of talk therapy, he joined The Ranch in 2017.

Dr. Fuller is passionate about helping people who have survived trauma and/or are struggling with addiction and strongly believes that the life we all deserve is within reach, no matter the adversity that one must overcome. He views medications as tools rather than solutions and his prescribing philosophy is to use the lowest dose necessary of non-addictive medications to help aid the process of recovery.
