Intensive Outpatient Program

The intensive outpatient program (IOP) at The Ranch Tennessee supports people who need consistent, structured care while continuing to live at home or in approved housing. This level of care is designed for individuals who are ready to apply recovery skills in real-world settings but still benefit from regular clinical guidance and accountability.

IOP is often a next step after residential or partial hospitalization, though some people begin care here based on their needs and history. The goal is to help clients strengthen daily routines, manage stressors as they arise, and build confidence outside of a residential environment.

Who IOP is designed for

IOP may be appropriate if you:

  • Are stepping down from residential or partial hospitalization programs
  • Need more structure than weekly therapy provides
  • Are managing substance use, mental health concerns, or both
  • Want to maintain work, school, or family responsibilities while receiving care

Each person’s schedule and care plan are individualized. Some attend programming during the day, while others participate in evening sessions, depending on availability and need.

What care looks like in IOP

Clients in IOP attend programming multiple days per week and participate in a combination of group sessions, individual therapy, and clinical check-ins. This structure allows people to practice skills in their daily lives while staying closely connected to the care team.

Programming focuses on:

  • Strengthening emotional regulation and distress tolerance
  • Recognizing patterns that contribute to relapse or instability
  • Building communication and boundary-setting skills
  • Developing practical strategies for managing triggers

When appropriate, clients may also participate in experiential elements connected to life at The Rach including equine-focused activities or outdoor-based practices. These experiences reinforce accountability, awareness, and follow-through—skills that translate directly into daily life outside The Ranch.

Trauma-informed care and clinical support

IOP at The Ranch is led by a trauma-informed clinical team with experience treating both substance use and primary mental health conditions. Care is grounded in understanding how past experiences influence current behavior and emotional responses.

When clinically appropriate, EMDR may be incorporated as part of a broader treatment plan. Not every client needs this approach, but having access to trained specialists allows care to be tailored rather than standardized.

Experiential support in IOP

While IOP is primarily a clinically focused level of care, The Ranch setting remains part of the experience. Clients may engage in equine-related work or outdoor practices that support grounding, emotional awareness, and responsibility.

Adventure-based elements such as ropes courses or zip line activities are introduced selectively and are designed to complement clinical goals, not replace them. Participation is based on readiness and clinical recommendation.

Staying connected after IOP

Planning for what comes next begins early. As clients progress, the care team helps coordinate ongoing outpatient services, alumni involvement, and community resources. The goal is sustained connection and support beyond formal programming.

Brian Fuller, MD

Medical Director

Dr. Fuller is board certified by the American Board of Psychiatry and Neurology. He completed medical school at the University of Tennessee Health Science Center and residency at Vanderbilt University. Additionally, he has completed the Nashville Advance Psychodynamic Psychotherapy Program, a two-year intensive program to better integrate traditional psychotherapy into his psychiatric practice. Dr. Fuller began his post-residency career in psychiatry running a psychotherapy-oriented private practice. Looking to help more people than he could working alone yet still provide treatment that emphasizes the importance and value of talk therapy, he joined The Ranch in 2017.

Dr. Fuller is passionate about helping people who have survived trauma and/or are struggling with addiction and strongly believes that the life we all deserve is within reach, no matter the adversity that one must overcome. He views medications as tools rather than solutions and his prescribing philosophy is to use the lowest dose necessary of non-addictive medications to help aid the process of recovery.
