What are compulsive sexual behaviors?

Compulsive sexual behaviors are patterns of sexual thoughts or actions that feel difficult to stop, even when they cause harm or go against personal values. These behaviors often become a way to cope with stress, emotional pain, or unresolved trauma.

They're more than just feelings of desire or being sexual. They’re part of a compulsive cycle fueled by a loss of control.

Common examples of compulsive sexual behaviors

Engaging with or pursuing multiple partners, even if in a relationship

Uncontrollable use of pornography, with or without visual stimuli

Obsessive sexual thoughts that interrupt daily life

Persistent fear of being found out or disappointing a loved one

Repeated attempts to stop that don’t last

Guilt and shame that lead to isolation and confusion

What causes compulsive sexual behaviors?

There is no single cause. For many men, compulsive sexual behaviors develop over time as a response to deeper issues.

The Sexual Recovery Program at Recovery Ranch Tennessee

The Sexual Recovery Program at The Ranch is a gender-specific program for adult men. It's certified by the International Institute for Trauma and Addiction Professionals (IITAP) and follows evidence-based practices for compulsive sexual behavior recovery.

The program takes place on our working ranch outside Nashville, Tennessee. The setting is quiet, rural, and intentionally structured to support focus and accountability.

A comprehensive approach

Our Sexual Recovery Program can be integrated with care for co-occurring substance use and mental health disorders. The clinical team at The Ranch works together to understand how these concerns interact, so recovery is approached as a whole rather than in pieces. This allows men to build stability across all areas of life, using consistent structure and shared skills throughout their stay. Treatment is available at these levels of care:

Why our approach to sexual recovery works

Our approach is built around structure, consistency, and understanding how trauma and the brain influence behavior.

The program utilizes Patrick Carnes’ Sexual Dependence Inventory (SDI) model, which helps clients:

  • Break through denial
  • Define clear sobriety and boundaries
  • Identify consequences of past behaviors
  • Develop awareness of patterns and triggers

Men are taught practical tools for emotional regulation, mindfulness, and coping with compulsive thoughts. Recovery here is skills-driven and reinforced through daily practice.

What to expect in the Sexual Recovery Program

Daily structure and environment

Days at The Ranch follow a steady rhythm. Structure reduces chaos and creates room for real work. Clients participate in therapy, group work, and outdoor activities that support grounding and connection.

The ranch setting matters. Open pastures, quiet mornings, and time outdoors help regulate the nervous system and support focus during recovery.

Clients are supported by a multidisciplinary team that includes:

  • Licensed therapists trained in trauma and compulsive sexual behaviors
  • On-site medical providers
  • Psychiatric professionals supporting mental and physical well-being
  • Marriage and family therapy professionals who guide family involvement

Therapeutic approaches we use

  • Dialectical behavior therapy (DBT)
  • Acceptance and commitment therapy (ACT)

Family support and involvement

We recognize the impact compulsive sexual behaviors have on families and partners. That’s why family engagement begins within the first 24 hours of treatment and continues throughout the stay.

  • Free weekly webinars for families led by marriage and family therapy experts

Experiential and holistic therapies

Recovery Ranch Tennessee is located outside Nashville on a working ranch. The land itself plays an active role in recovery.

Clients engage in:

  • Equine therapy, working with horses to build trust, emotional awareness, and non-verbal communication
  • Experiential therapy, including ropes courses and outdoor challenges that build resilience and teamwork
  • Daily movement and time outdoors, supporting regulation and reflection

Housing is gender-separate, allowing our clients to focus on their own work while building connections with other men walking similar paths.

Brian Fuller, MD

Medical Director

Dr. Fuller is board certified by the American Board of Psychiatry and Neurology. He completed medical school at the University of Tennessee Health Science Center and residency at Vanderbilt University. Additionally, he has completed the Nashville Advance Psychodynamic Psychotherapy Program, a two-year intensive program to better integrate traditional psychotherapy into his psychiatric practice. Dr. Fuller began his post-residency career in psychiatry running a psychotherapy-oriented private practice. Looking to help more people than he could working alone yet still provide treatment that emphasizes the importance and value of talk therapy, he joined The Ranch in 2017.

Dr. Fuller is passionate about helping people who have survived trauma and/or are struggling with addiction and strongly believes that the life we all deserve is within reach, no matter the adversity that one must overcome. He views medications as tools rather than solutions and his prescribing philosophy is to use the lowest dose necessary of non-addictive medications to help aid the process of recovery.
